JOB SUMMARY:
Provide patient education to individuals and groups of patients with diabetes and to their support person(s). Utilize the nursing process in assessment, planning, intervention, and evaluation of educational needs in coordination with the patient's physician and other members of the health care team.
QUALIFICATIONS:
Education/Training:
- Graduation from a school of nursing is required.
- Certified Diabetic Educator (CDE) certification required.
- Bachelor of Science in Nursing (BSN) is preferred.
Experience: Two or more years of nursing experience and a minimum of 520 hours working with individuals with Type I, Type II, and Gestational diabetes.
